Logon to IT Service Management Suite.
Select RequestIT from the menu on the left.
Select IT Services by clicking on the checkbox.
Select Service Center Support Assessment by clicking on the checkbox.
Click on the Choose Options button.
Enter specific information about the project in the Notes Comments section. Include the project name, the system name if known, a brief description of the goal of the project, the purpose of the system and the anticipated go-live date.
