Request, complete and return the completed Application Registration Assessment to the Information Security Specialist to add attributes to the AppProfile report.

The Project Manager receives and completes the Application Registration Assessment after informing Information Secuirty of the need to add an application to the Application Lifecycle Process.  Information Security receives the completed application registration and adds the application attributes to the AppProfile Report.

Steps
PM notifies Information Security of the need to add an application to the Application Lifecycle Process
IS Specialist sends the Project Manager the Application Registration
Project Manager completes the Application Registration and returns to Information Security
IS Specialist adds application attributes to the Application Profile Report
Application Owner, Technical Manager and surrogates are assigned based upon data from the Application Registration
